CNA Skills – Bed Shampoo

CNA Skills – Bed Shampoo is a thorough cleansing of the hair and scalp with thick scalp lather applied with friction and rinsed with clean water. This procedure is done in bed.

The purpose of bed shampoo are the following:

  1. To cleanse the hair ans scalp
  2. To refresh the patient
  3. To complete treatment of pediculi
  4. To promote physical and mental comfort of the patient.

Precautions and Special Considerations in performing Bed Shampoo:

  1. Be sure the treatment is ordered by the doctor
  2. Avoid exposure of the patient and chilling
  3. Observe patient for sign of exhaustion
  4. Provide comfort for the patient during the procedure
Procedure Key Points
1. Bring all equipment needed near the bed To save time and motion
2. Screen the patient, close windows if necessary To provide privacy
3. Explain the procedure to the patient; take and record patient’s TPR To gain his cooperation
4. Loosen top sheet a and assist patient assume comfortable position To facilitate movement
5. If Kelly pad is available, protect bed with rubber sheet, use Kelly pad dropping free into pail. Place rolled edge of Kelly pad covered with towel under the patient’s head For comfort
6. Comb and brush patient’s hair and observe condition of scalp
7. Cover patient’s eyes with washcloth To avoid the soap from getting into the patient’s eyes
8. Plug patient’s ears with cotton To avoid water from getting into patient’s ears
9. Fill small pitcher with water Temperature may depend on patient’s tolerance
10. Pour warm water gradually over the hair wetting it thoroughly. Direct water away from patient’s ears and eyes
11. Pour small quantity of shampoo solution into palm of hand and apply it on hair
12. Massage scalp thoroughly with finger tips, with firm circular motions working up a good lather To bring out the natural oil
13. Rinse well To prevent dandruff
14. Repeat until hair is sufficiently clean
15. Rinse until hair is free from soap
16. Remove Kelly pad and drop it into pail
17. Cover the patient’s head with bath towel which was used to protect the neck and rub hair well To remove excess moisture
18. Place another bath towel under patient’s head
19. Comb hair and allow it to dry
20. When hair is dry, assist patient in fixing it

After Care:

  1. Take all used equipment
  2. Wring water from used towels and put them together with other soiled linens in the linen hamper
  3. Wash pitcher, pail, enamel and Kelly pad a with soap and water. hang Kelly pad to dry

CNA Skills – Taking an Oral Temperature

The following precautions should be observed when taking an oral temperature:

  1. Do not take temperature if patient has had a cold or hot drink or after smoking (take it after 30 minutes)
  2. Do not take temperature by mouth when the patient is:
    • Child
    • Unconscious
    • Restless
    • Delirious
    • Hysterical
  3. Do not take after oral and nasal surgery

Procedure

Key Points

1. Clean the digital thermometer’s tip with cool, soapy water, or wipe it with 70 percent isopropyl alcohol. To remove germs or bacteria
2. Choose the right or proper thermometer. The American Association of Pediatrics recommends using a digital thermometer and stay away from the use of mercury thermometers, which can be toxic. Choose a digital pacifier thermometer if the patient is a child between 3 months and 4 years old and a regular digital thermometer for children over 4 years old. Mercury may be fatal if inhaled and harmful if absorbed through the skin. Around 80 percent of the inhaled mercury vapor is absorbed in the blood through the lungs. It is harmful to the nervous, digestive, respiratory and immune systems. Exposure to mercury can cause tremors, paralysis, impaired vision and hearing, insomnia, attention deficit, and developmental delays during childhood.
3. Have the patient at rest (seated or lying down) and place the thermometer under the patient’s tongue and ask the patient to keep mouth closed for three minutes or until the thermometer beeps and his temperature appears on the digital screen The average oral temperature for children ranges from 97.6 to 99.3 degrees Fahrenheit (36.4 to 37.4 degrees Celsius). While an adult’s normal temperature is 98.6 degrees Fahrenheit (37 degrees Celsius); an elderly person’s temperature could be as low as 96.8 degrees Celsius (36 degrees Celsius).

Procedures

Key Points

1. Turn on water to a comfortable temperature Avoid contaminating faucet by using paper towel when turning in and off faucet. If faucets are controlled by foot pedal or elbow levers, disregard paper towel
2. Wet your hand and apply enough soap to make heavy lather, rub vigorously, using one hand first and then another. Wash thoroughly for one (1) minute for each hand.
3. Rinse under running water
4. Repeat procedure no.2 for another hand
